DPH Home Visiting Program

(Perinatal Health Partnership)

The DPH Home Visiting Program provides home visiting services during pregnancy and up to the infant’s first birthday following delivery. Pregnant women with high-risk conditions or have identified risk factors that put them at risk for poor pregnancy outcomes, will be targeted for home visiting services and interventions but enrollment can occur after delivery.

The DPH Home Visiting Program provides additional monitoring between provider appointments that will increase the opportunity to identify warning signs, complications, and problems earlier and refer to provider for further assessment and intervention when indicated.
