What is a Community Health Profile (CHP)?

A Community Health Profile is a process by which health data was consolidated and analyzed at various geographic levels to understand place-based inequities and to inform needs assessments, community planning, and resource allocation.

Why assess the health of our community?

The Community Health Profile of the Coastal Health District provides a comprehensive overview of the health landscape in the region. Through the analysis of demographic data, healthcare resources, and various health indicators, the profile offers valuable insights into the challenges and strengths of our community. The compilation of this data is useful for directing and guiding the prioritization of community health needs within the Coastal Health District.

Community Health Profile Dashboard: This interactive dashboard informs members of the Coastal Georgia community about the health indicators of their county, as well as access to visualize and evaluate the health of their community. OASIS: The Online Analytical Statistical Information System is the Georgia Department of Public Health’s data warehouse. This suite of interactive tools allows users to query health data for a variety of purposes, including for analysis of the health of a region, grant writing, and simply general interest in the health of your local community.
